You shouldn’t have to be an AI expert.
The planned experience includes an assistant. No separate AI subscription or technical setup should be needed to get started.
A health record built around you
You’ve done the appointments. Kept the notes. Tried to connect the dots. You shouldn’t have to keep starting over.
We’re building a patient-owned health record that brings your medical history together with your journals, wearables, and everyday surroundings. Build on what you’ve already learned. Choose who helps you make sense of it.

The people and assistants you invite
Your history stays with you.
You decide who has access.
Brought together in your record
For the history you’ve already lived
The specialist who hasn’t seen your earlier results. The treatment you tried two years ago. The symptoms that never made it into a visit note. You’re the one carrying the connections.
That work should count. Our goal is a record that brings it forward—so the next conversation can build on your history, and you can advocate for yourself with the context at hand.
The approach
The record we’re working toward:
built around your life,
available
on your terms.
The vision is to bring records from your doctors’ systems together with what only you can add: symptoms, journals, wearable readings, and the conditions around you.
An included assistant will help you explore that history, keep track of open questions, and identify useful gaps. The aim is to make your existing information work harder for you.
We’re designing for sharing on your terms—with a doctor, a second opinion, or an AI assistant you trust. Your record should remain useful as your care team changes.
Less work to keep it together
You have enough to manage.
The software should meet you where
you are.
The planned experience includes an assistant. No separate AI subscription or technical setup should be needed to get started.
Recollections are estimates. We’re designing for quick answers and ordinary language, with uncertainty kept in view.
Remember something differently? The goal is to let you say so in conversation, and have the assistant connect the correction to the right information.
Ownership is the point
Our ambition is a complete health record owned by the patient: clinical records alongside the lived experience that a medical chart can miss.
You should decide who can access your information and what they can see. Doctors and AI assistants can help you work with it; changing either should leave your history with you.
That means designing for export, control over access, and evidence that stays linked to its source. Patient ownership needs to be something you can exercise.
End-to-end privacy is a long-term design goal, not a capability we’re claiming today. This preview site does not accept health records or create patient accounts.
Before you begin
Not yet. We’re developing the first experience: bringing information together, asking useful follow-up questions, and making ongoing collection simpler. This page is a preview of that direction.
No AI expertise should be needed for the experience we’re building. The plan is to include an assistant you can talk to naturally. Connecting your own assistant will be an optional path.
We’re building a tool for organizing personal information and exploring health questions. It is not a substitute for medical care, and an apparent pattern is not a diagnosis. The aim is to help you advocate for yourself in more informed conversations with your clinician.
The long-term vision brings together records from doctors’ EHRs, labs, treatments, journals, wearables, and environmental context. We’re starting with lab reports and personal observations, with an iPhone companion and selected Apple Health data planned. Provider connections and sharing will arrive in stages; not every source will be supported at launch.
This preview has no account registration, health uploads, analytics scripts, or advertising trackers. Fonts are hosted with the site. Our hosting provider processes connection information to deliver the page; that is separate from the future product’s data practices.
